End Homelessness Vermont is launching a program to help ensure that our neighbors and community members experiencing homelessness are truly supported. More than 70 of our clients living with complex needs have made it home in the last 12 months. Help us double that this year!
Help Your Neighbors Make It HomeEnd Homelessness Vermont works primarily with people living with complex medical needs and disabilities whom often have the highest barriers to finally making it home. In the last few years, people have experienced endless exits to unsheltered homelessness and these have caused extensive setbacks in access to supports, services and their housing search. Every moment of progress lost while unsheltered, means a longer time in need of homelessness supports. It is in our clients and our states best interest that we ensure that there is a seamless transition to housing from homelessness as the first and most primary need, while being careful to have a client centered approach, ensuring services are appropriately navigated, medical needs are met along the way and the systems that are supposed to help people are not harming them. This is why we have initiated the Making It Home fund, to ensure that our neighbors will have all the support they need from the moment of emergency into permanent housing and beyond. End Homelessness Vermont has a 99.8% track record in keeping clients engaged in our services. When we don't leave people behind, we can end homelessness in Vermont.

The Make It Home Program represents End Homelessness Vermont's focused commitment to addressing a critical gap: ensuring that people with the highest needs, both medical and living with disabilities at the intersection of homelessness and housing insecurity receive the specialized, responsive support they need to achieve housing stability.

Make It Home brings together End Homelessness Vermont's proven strategies under one coordinated program designed specifically for Vermont communities. This program is replicable in states across the country, using the unique barriers, services and systems that each state has.

While End Homelessness Vermont hotline is available for all Vermonters experiencing homelessness, and we work primarily with people living with disabilities every day. Make It Home concentrates a specialized program dedicated to those with the highest need specifically on complex barrier-related housing challenges, ensuring clients receive support tailored to their circumstances.
Collaborative Team Approach
Make It Home brings together the full strength of End Homelessness Vermont's team alongside each client's providers across the region. This collaborative approach ensures that healthcare providers, benefits specialists, housing providers, disability services, and other key partners work together seamlessly to meet the unique needs of each household.
Coordinated Support Model
Make It Home integrates comprehensive non categorical case management, immediate shelter solutions, housing placement, and service navigation into one seamless program. Clients work with dedicated case managers and systems navigators who understand disability rights, accessible housing requirements, and the specific systems this community navigates. Within End Homelessness Vermont, our team also uses a team approach so that all team members are able to work through complex barriers.
No Wrong Door, No Gaps in Shelter
The program has the goal of ensuring that clients maintain shelter throughout their housing search, regardless of traditional or emergency shelter capacity. This commitment means people aren't cycling between the streets and temporary options—they have stability while working toward permanent solutions.

Make It Home doesn't just find any available unit. The program prioritizes housing that is truly accessible, affordable, and keeps people in their communities and support networks that matter to them.
